ABSTRACT

Abstract Objectives: Miniscrew has been frequently used, considering that anchorage control is a critical point in orthodontic treatment, and its failure, the main adverse problem. Using two groups of stable (successful) and unstable (failed) mini-implants, this in vivo study aimed to quantify proinflammatory cytokines IL-1 α, IL-6, IL-17, and TNF-α and osteoclastogenesis marker RANK, RANKL, and OPG in gingival tissue, using the real-time polymerase chain reaction technique. Methodology: Thirteen patients of both sexes (11-49 years old) under orthodontic treatment were selected, obtaining 11 successful and 7 failed mini-implants. The mini-implants were placed and removed by the same surgeon, in both jaws. The mean time of permanence in the mouth was 29.4 months for successful and 7.6 months for failed mini-implants. At removal time, peri-mini-implant gingival tissue samples were collected and processed for quantification of the proinflammatory cytokines and osteoclastogenesis markers. Nonparametric Wilcoxon rank-sum test considering the clusters and Kruskal-Wallis test were used for statistical analysis (α=0.05). Results: No significant difference (p>0.05) was observed between the groups for either quantification of cytokines or osteoclastogenesis markers, except for IL-6 (p<0.05). Conclusions: It may be concluded that the expression of IL-1α, IL-17, TNF-α, RANK, RANKL, and OPG in peri-implant gingival tissue were not determinant for mini-implant stability loss, but the higher IL-6 expression could be associated with mini-implant failure.

ABSTRACT Objective: The objective of this study was to assess the external apical root resorption (EARR) of the maxillary posterior teeth after intrusion with miniscrews. Methods: Fifteen patients (13 females and 2 males) with age ranging from 14.5 to 22 years (mean 18.1 ±2.03 years) were selected to participate in this study. All patients presented with anterior open bite of 3 mm or more. An intrusion force of 300 g was applied on each side to intrude the maxillary posterior teeth. Cone beam computed tomography (CBCT) scans were taken pretreatment and post-intrusion and were analyzed to evaluate the EARR. Results: The maxillary posterior teeth were intruded in average 2.79 ± 0.46 mm (p< 0.001) in 5.1 ± 1.3 months, and all examined roots showed statistically significant EARR (p< 0.05) with an average of 0.55 mm, except the distobuccal root of the left first permanent molars and both the palatal and buccal roots of left first premolars, which showed no statistically significant changes. Conclusions: The evaluated teeth presented statistically significant EARR, but clinically, due to the small magnitude, it was not considered significant. Moreover, the CBCT provided a good visualization of all roots in all three planes, and it was effective in detecting minimal degrees of EARR.


RESUMO Objetivo: o objetivo desse estudo foi verificar a existência de reabsorção radicular apical externa (RRAE) em dentes posterossuperiores após intrusão ancorada em mini-implantes. Métodos: quinze pacientes (13 mulheres e 2 homens) com a idade variando entre 14,5 e 22 anos (média de 18,1 ± 2,03 anos) foram selecionados para participar desse estudo. Todos os pacientes possuíam mordida aberta anterior de 3mm ou mais. Uma força de 300 gramas foi aplicada em cada lado para intruir os dentes posterossuperiores. Tomografias computadorizadas de feixe cônico (TCFC), adquiridas antes do tratamento e após a intrusão, foram comparadas para se avaliar a RRAE. Resultados: os dentes posterossuperiores foram intruídos em média 2,70 ± 0,46 mm (p< 0,001) em 5,1 ± 1,3 meses, e todas as raízes examinadas mostraram RRAE estatisticamente significativa (p< 0,05), com média de 0,55 mm, exceto pela raiz distovestibular dos primeiros molares permanentes esquerdos, e pelas raízes palatina e vestibular dos primeiros pré-molares esquerdos, que não apresentaram mudanças estatisticamente significativas. Conclusões: os dentes avaliados apresentaram RRAE estatisticamente significativa, a qual, porém, não foi considerada clinicamente significativa, devido à sua reduzida magnitude. Além disso, a TCFC possibilitou uma boa visualização de todas as raízes nos três planos espaciais, e foi eficaz para detecção de níveis mínimos de RRAE.

ABSTRACT Objective: The aim of his study was to evaluate the stress on tooth and alveolar bone caused by orthodontic intrusion forces in a supraerupted upper molar, by using a three-dimensional Finite Element Method (FEM). Methods: A superior maxillary segment was modeled in the software SolidWorks 2010 (SolidWorks Corporation, Waltham, MA, USA) containing: cortical and cancellous bone, supraerupted first molar, periodontal tissue and orthodontic components. A finite element model has simulated intrusion forces of 4N onto a tooth, directed to different mini-screw locations. Three different intrusion mechanics vectors were simulated: anchoring on a buccal mini-implant; anchoring on a palatal mini-implant and the association of both anchorage systems. All analyses were performed considering the minimum principal stress and total deformation. Qualitative analyses exhibited stress distribution by color maps. Quantitative analysis was performed with a specific software for reading and solving numerical equations (ANSYS Workbench 14, Ansys, Canonsburg, Pennsylvania, USA). Results: Intrusion forces applied from both sides (buccal and palatal) resulted in a more homogeneous stress distribution; no high peak of stress was detected and it has allowed a vertical resultant movement. Buccal or palatal single-sided forces resulted in concentrated stress zones with higher values and tooth tipping to respective force side. Conclusion: Unilateral forces promoted higher stress in root apex and higher dental tipping. The bilateral forces promoted better distribution without evidence of dental tipping. Bilateral intrusion technique suggested lower probability of root apex resorption.


RESUMO Objetivos: o presente estudo teve como objetivo avaliar, por meio da Análise de Elementos Finitos, as tensões geradas por forças ortodônticas intrusivas em um molar superior e no osso alveolar circundante. Métodos: um segmento maxilar foi modelado no software SolidWorks 2010 (SolidWorks Corporation, Waltham, MA, EUA), contendo: osso cortical e medular, primeiro molar extruído, tecido periodontal e acessórios ortodônticos. Um modelo de elementos finitos simulou forças intrusivas de 4 N no elemento dentário, aplicadas a partir de mini-implantes em localizações distintas. Três diferentes vetores para mecânica de intrusão foram simulados: ancoragem em mini-implante vestibular, ancoragem em mini-implante palatino ou a associação de ambas. Todas as análises foram realizadas em termos de tensão mínima principal e deformação total. A análise qualitativa foi feita por meio do mapeamento da distribuição das tensões em gradiente de cores. A análise quantitativa foi feita em software específico para leitura e resolução de equações numéricas (ANSYS Workbench 14, Ansys, Canonsburg, Pennsylvania, EUA). Resultados: as forças intrusivas aplicadas simultaneamente nos dois lados (vestibular e palatino) resultaram em uma distribuição mais homogênea das tensões geradas, sem zonas de acúmulo de tensão e com uma resultante vertical para a movimentação intrusiva. As forças aplicadas somente em um lado, vestibular ou palatino, resultaram em zonas de concentração de tensão, com maiores valores, e na inclinação do elemento dentário para o lado em que a força foi aplicada. Conclusão: as forças unilaterais promoveram maior tensão no ápice radicular e maiores inclinações do elemento dentário. Já as forças bilaterais promoveram melhor distribuição das tensões e não resultaram em inclinação do elemento dentário. Assim, as forças intrusivas ancoradas bilateralmente apresentam menor probabilidade de reabsorção do ápice radicular.

Este trabalho teve como objetivo avaliar o torque e a carga axial de inserção e a severidade das lesões radiculares em dentes humanos decorrentes da utilização de diferentes diâmetros de mini-implantes com finalidade ortodôntica. Sessenta pré-molares extraídos foram inclusos em osso artificial e divididos em 3 grupos, correspondentes a 3 diferentes diâmetros de mini-implante (1,4mm, 1,6mm e 1,8mm). Em cada raiz dentária foram inseridos manualmente 2 mini-implantes autoperfurantes utilizando dois métodos de inserção (com e sem perfuração prévia inicial com fresa) e em duas alturas (apical e cervical). Quinze mini-implantes foram inseridos apenas em osso artificial como parâmetro de comparação. Foram quantificados o torque e a carga axial de inserção através de um torquímetro axial digital e uma célula de carga respectivamente. Os dentes foram, então, escaneados e analisados através de microtomografia computadorizada (Micro-CT) a fim de se observar as severidades das lesões radiculares provocadas pelos mini-implantes. Os dados referentes ao torque e à carga axial dos mini-implantes inseridos em pré-molares foram submetidos à análise de variância (ANOVA) a três critérios, sendo aplicados posteriormente testes de Tukey para comparação entre os grupos. A diferença dos torques e das cargas axiais entre os mini-implantes inseridos no osso artificial e nas raízes dentárias foi estimada por testes t. Foram aplicados também testes de Qui-Quadrado para avaliar as severidades das lesões radiculares. Tanto os torques como as cargas axiais de inserção dos mini-implantes inseridos em raízes de dentes humanos apresentaram diferenças estatisticamente significantes entre os diferentes diâmetros avaliados, porém sem um padrão definido. Sixty extracted premolars were included in artificial bone and divided into three groups according to different mini-implant diameters (1.4mm, 1.6mm and 1.8mm). Two self-drilling mini-implants were manually inserted into each root using two placement methods (with and without initial pre-drilling) in the apical and cervical regions. Fifteen mini-implants were inserted into artificial bone as a comparison parameter. Placement torque and axial placement load were quantified by a digital torquimeter and a load cell, respectively. The teeth were scanned and analyzed with microcomputed tomography (Micro-CT) to observe the severity of root lesions caused by the mini-implants. The torque and the axial load of mini-implants inserted into premolars were analyzed by 3-way analysis of variance (ANOVA), followed by Tukey tests. Differences in the torques and axial loads between the mini-implants inserted in dental roots and artificial bone were compared with t tests. Chi-square tests were applied to assess the severity of root lesions. The placement torques and the axial placement loads of mini-implants inserted into human dental roots showed significant differences between the different diameters evaluated, but with no standardized effect. Este reporte de caso describe el tratamiento quirúrgico de un primer molar maxilar como solución a la perforación iatrogénica de su raíz mesiovestibular tras la colocación de un microtornillo para el refuerzo del anclaje ortodoncico en un paciente adulto. El objetivo de este caso clínico es dar a conocer una de las posibles complicaciones asociadas a la colocación de microtornillos interradiculares asociados al refuerzo del anclaje en ortodoncia, su diagnóstico y tratamiento.


The aim of this publication is to present one of the possible complications associated with the placement of interradicular miniscrews for the reinforcement of anchorage in orthodontics. This case report describes the surgical treatment of a maxillary first molar as a solution for the iatrogenic perforation of its mesiobuccal root after the placement of a microscrew for the reinforcement of the orthodontic anchorage in an adult patient.

Background: Pain and discomfort are common during orthodontic treatment. Aim: The aim of this single blind clinical trial was to compare the effectiveness of oral calcium versus acetaminophen in pain reduction. Materials and Methods: In this study, the patients recorded their pain on a 100-mm visual analogue scale (VAS). Forty female patients (14-19 years old) who had passed at least 2 months of their first orthodontic archwire placement and their pain intensity was 40-100 mm (VAS) were selected and randomly assigned to two groups. Psychotic status was measured using Hospital Anxiety and Depression Scale (HADS). In group 1 calcium forte tablets (500 mg) and in group 2 acetaminophen (325 mg) tablets were prescribed to be taken one tablet per day. After consumption of all tablets (n=60), pain intensity was measured and compared with that before drug therapy. Results: The results indicated that the difference in pain intensity before and after drug administration in the calcium group was statistically significant (P<0.001), but not significant in the acetaminophen group (P=0.468). The difference between the pain reduction in the two groups were also statistically significant (P<0.001). In the calcium group 9.5% and in the acetaminophen group 15.8% had anxiety that was not statistically significant (P=0.631). In both groups, no subject had depression. Conclusion: Calcium is more effective than acetaminophen in long-term pain reduction during orthodontic treatment.

Background: Orthodontic forces may not only influence the dentoalveolar system, but also the adjacent and surrounding cortical bone. Aim: Since there is very limited information on this issue, we aimed to study the possible changes in maxillary cortical bone following the application of heavy orthodontic forces in mature normal and osteoporotic rats. Materials and Methods: Twenty-four 6-month-old female rats were selected and divided into an ovariectomized group and a normal group. In both groups, the rats were subjected to a 60 gr* orthodontic force on the upper right first molar for 14 days. Results: In both groups, histological sections showed that the application of this force caused hypertrophy and fatigue failure of the cortical maxillary bone. The osteogenic reaction to distraction is expressed by the formation of subperiosteal callus on the outer bony side, resembling that seen in distracted bones. Conclusion: From this study we concluded that heavy experimental orthodontic forces in rats affect the maxillary cortical bone. The osteogenic reaction to these forces, expressed histologically by subperiosteal callus formation, is similar to that seen in distraction osteogenesis models.

Purpose: The objective of this study was to evaluate and compare the perceived pain intensity, side effects and discomfort related to the moment of placement, during mechanics and removal of a mini-screw for molar distalization in orthodontic treatment.Methods: The sample consisted of 30 adult patients with a mean age of 30 years old, with class II malocclusion subdivision right or left. A mini-screw was installed in each patient, in the maxillary arch to provide a molar distalization. The patients answered a questionnaire to assess their opinions on the treatment. Results: Ninety percent of the patients chose mini-screws over pre-molar extraction (orthodontic camouflage), or the use of an extra-oral appliance (Kloehn cervical traction) or another non-compliance treatment to class II. Aphthous ulcer was the side effect most frequent after placement of the mini-screw (30%). The greatest discomfort was felt during infiltration anesthesia (27%), though 23% reported no discomfort during placement. Eighty-three percent of the patients reported no pain during placement, which may be associated with the degree of satisfaction with the treatment (100%) and recommendation of the procedure to other patients (100%). Conclusion: Mini-screws were well accepted by the patients and were efficient for molar distalization when utilized in association with orthodontic treatment for Class II malocclusion.


Objetivo: O objetivo desse estudo foi avaliar e comparar a intensidade de dor, os efeitos colaterais e o desconforto sentido pelo paciente relacionado à colocação do mini-implante, durante a mecânica e na remoção do mesmo, durante a distalização de molares no tratamento ortodôntico. Métodos: A amostra foi composta por 30 pacientes adultos com uma média de idade de 30 anos, com maloclusão Classe II subdivisão direita ou esquerda. Um mini-implante foi instalado em cada paciente, no arco superior para conseguir a distalização de molar. Os pacientes receberam um questionário a respeito de sua opinião sobre o tratamento. Resultados: Noventa por cento dos pacientes escolheram colocar os mini-implantes ao invés de realizar extração de molares (camuflagem ortodôntica), fazer o uso de um aparelhoextraoral (Aparelho extraoral de Kloehn) ou fazer uso de qualquer outro tratamento alternativo para classe II. A úlcera aftosa foi o efeito colateral com a maior incidência após a inserção do mini-implante (30%). O maior desconforto foi sentido durante a infiltração anestésica (27%), embora 23% relataram não ter sentido desconforto durante o tratamento. Um total de 83% dos pacientes não relatou dor durante o tratamento, o que pode ser associado com o grau de satisfação com o tratamento (100%) e a recomendação desse procedimento a outros pacientes (100%). Conclusão: Os mini-implantes foram bem aceitos pelos pacientes e foram eficientes para a distalização de molares quando utilizados em associação com o tratamento ortodôntico para correção da maloclusão de Classe II.

ABSTRACT

Los implantes de titanio como anclaje para el movimiento dentario en la práctica ortodontica es una terapia en constante crecimiento en consecuencia del poco tiempo de uso. De esta forma, sus complicaciones no han sido completamente evaluadas. El presente caso representa una complicación asociada a una técnica incorrecta, acentuada por algunas características locales. La resolución final del caso es simple y responde a la indicación quirúrgica (biopsia excisional) para este tipo de situaciones.


Titanium implant used for anchorage to dental movement in orthodontics practice is a growing therapy because its shorter treatment time. Because of this, its complications have not been totally evaluated. The present case reports a complication due an incorrect technique, increased by characteristic of the area. The final resolution of this problem is simple and responses for the surgical indications (excisional biopsy) in this situation.
